WHAT IS PERSONAL INFORMATION?
Personal Information pertains to recorded information about an identifiable individual. This information may include an individual’s name, home address, phone number, age, sex, marital or family status, and identifying numbers, financial information, educational history, etc. We will treat all personal information provided to the PHWDG as sensitive and confidential.
Personal information relating our partners and stakeholders will be held in the strictest confidence.
In consultation with the Board of Directors, the Executive Director is responsible for the creation of internal guidelines and procedures for the collection, use, disclosure, storage and destruction of personal information. All employees are required to protect the confidentiality of personal information.
We collect, use or disclose information primarily to:
> Communicate information to you;
> Inform you of PHWDG activities and programs;
> Invite you to a PHWDG event; and/or
> Invite you to participate in research projects.
We will communicate to individuals, before or at the time of collecting personal information, of the purposes for which we are collecting the information.
We will never sell, trade or lease your personal information and we will never share personal information without your prior consent. We will make every possible effort to prevent loss, misuse, disclosure or modification of personal information as well as prevent any unauthorized access to personal information.
